What Is Breast Ptosis?

Breast ptosis is the medical term for breasts that have begun to droop or sit lower on the chest. It is typically evaluated by looking at the position of the nipple in relation to the natural breast crease. The nipple may remain above the crease in mild cases or fall below it as sagging becomes more pronounced.

Some patients experience pseudoptosis, where the nipple remains near the breast crease but more of the breast tissue settles below it. This distinction matters because breast shape is not determined by skin alone.

Why Breast Sagging Develops

Several changes can contribute to breast sagging over time:

•          Reduced skin elasticity as collagen and supportive tissues weaken

•          Pregnancy and breastfeeding-related expansion and volume changes

•          Significant weight gain or weight loss

•          Natural aging and hormonal changes

•          Genetics, breast size, and the effects of gravity

These factors can cause the skin envelope to loosen while the internal breast tissue shifts downward. The upper portion of the breast may also appear flatter or less full, especially after pregnancy or weight loss.

Stretched Skin Versus Loss of Breast Position

Stretched skin creates a looser outer envelope, but loss of position involves the breast tissue and nipple sitting lower on the chest. Most patients have a combination of both.

A breast lift, also called mastopexy, is designed to remove excess skin, reshape the underlying breast tissue, and reposition the nipple and areola. The procedure focuses on improving breast position and contour rather than significantly changing breast size.

When Volume Loss Is Also Present

Breast sagging may occur alongside a loss of upper breast fullness. In these cases, a breast lift may improve position but may not restore the desired amount of volume.
